FLEXIBLE SCOPE REPAIR

Instructions

For Flexible Scope/s being sent for estimate / repair, please follow these steps:

1. Review the packing slip to confirm accurate shipping address
   
   
2. Be sure to disinfect all medical devices
   
   
3. Print out the completed packing slip and place in the box with the item/s 
   
   
4. Send the equipment previously authorized matching the packing slip
   
   
5. Be certain to securely pack the item/s being sent in for repair to minimize chance of further damage during shipping
   
   
6. Please email the tracking number for the item being shipped for repair to pieri@eriemedsupplies.com
   
   
7. Estimates will be provided and must be approved prior to repair - All scope repairs must be prepaid.
